What is the best treatment for Zollinger-Ellison syndrome?

Medications known as proton pump inhibitors are the first line of treatment. These are effective medications for decreasing acid production in Zollinger-Ellison syndrome. Proton pump inhibitors are powerful drugs that reduce acid by blocking the action of the tiny «pumps» within acid-secreting cells.

Does Gastrinoma cause secretory diarrhea?

Gastrinomas are neuroendocrine tumors characterized by the secretion of gastrin with resultant excessive gastric acid production causing severe peptic ulcer disease and diarrhea, a combination referred to as the Zollinger-Ellison syndrome (ZES).

How does Zollinger-Ellison syndrome cause Steatorrhea?

Steatorrhea: This defect occurs in part because inactivation of pancreatic lipase by intraluminal acid in the upper small intestine and the low pH environment render some primary bile acids insoluble, and thereby reduce the formation of micelles (which are necessary for fatty acid and monoglyceride absorption).

How do you get rid of Zollinger-Ellison syndrome?

Treatment

  1. Removing as much of a liver tumor as possible (debulking)
  2. Attempting to destroy the tumor by cutting off the blood supply (embolization) or by using heat to destroy cancer cells (radiofrequency ablation)
  3. Injecting drugs into the tumor to relieve cancer symptoms.
  4. Using chemotherapy to try to slow tumor growth.

Is Zollinger-Ellison syndrome curable?

The condition can be cured if the gastrinoma is successfully removed with surgery. If surgery is not possible, in some cases Zollinger-Ellison syndrome can be managed medically. People with Zollinger-Ellison syndrome can live active lives for many years after diagnosis.

What type of diarrhea does Gastrinoma cause?

Excessive HCl production also causes hyperperistalsis, a condition marked by excessive rapidity of the passage of food through the stomach and intestine and inhibits the activity of lipase, causing severe fatty diarrhea known as steatorrhea.

Why is steatorrhea a common manifestation of Zollinger syndrome?

Diarrhea is common because the large-volume gastric acid output leads to structural damage to the small intestine (inflammation, blunted villi, edema), interference with fat transport, inactivation of pancreatic lipase, and precipitation of bile acids. These same mechanisms, if prolonged, can lead to steatorrhea.
